Front-end, mid-cycle, and back-end

Revenue cycle management

Revenue cycle support in the three stages ForeMed uses: front-end, mid-cycle, and back-end. Payment posting and charge entry are stated at a 24-hour turnaround.

ForeMed supports the revenue cycle from eligibility and benefits through payment, credit balance, and denial follow-up. Medical coding is a separate service that feeds the mid-cycle. It can be engaged on its own.

Work is done in your process and your systems. We do not replace your practice-management or hospital billing system, and we do not quote a collection rate.

What we support

01

Eligibility and benefit verification

Front-end support for coverage and benefit checks before or at the time of service.

Gaps that would hold a claim — inactive coverage, a missing subscriber detail, a benefit that does not match the planned service — are flagged rather than passed downstream.

02

Prior authorization

Front-end support for prior authorisation preparation and status follow-up, using the payer rules and clinical information you provide.

We do not invent clinical rationale. If the record is short of what the payer asks for, the item is returned.

03

Charge posting and validation

Mid-cycle support for charge entry and for validation of those charges against the coded encounter.

Charge entry is one of the two tasks ForeMed states at a 24-hour turnaround. The clock and the exceptions belong in the statement of work.

04

Billing and edit resolution

Mid-cycle support for billing edits: demographic, coding, and payer-rule edits that can be cleared from the information in the claim and the record.

An edit that needs a clinical decision is returned with the edit text. It is not overwritten.

05

Payment posting

Back-end support for posting payments and adjustments to the account.

Payment posting is the other task stated at a 24-hour turnaround. Unmatched payments are queued for review rather than forced onto an account.

06

Credit balance

Back-end support for finding credit balances and for the resolution steps your policy allows, including the distinction between a true overpayment and a posting error.

07

Denial management and accounts receivable

Back-end support for denial management and A/R follow-up. Appeals are prepared when the documentation and the payer rule support them.

Each open item keeps a status: what was done, what is waiting, and what cannot be appealed. We do not describe aged inventory as collected.

How a revenue cycle engagement runs

  1. 01

Map the cycle you already run

Front-end, mid-cycle, and back-end tasks are listed against your systems, payers, and specialties. Coding stays linked, not duplicated.

  2. 02

    Write the instruction

Turnaround, exception paths, and who may adjust an account are written down and signed off before production.

  3. 03

    Run the agreed tasks

Eligibility, authorisation, charge entry, edits, posting, credit balance, and denial follow-up, only where the instruction includes them.

  4. 04

    Review the sample

The quality practice for this work is the same one stated for ForeMed’s production: a dedicated quality team, a 3–5% daily audit sample, SOP sign-off, and calibration.

Deliverables

  • Completed front-end, mid-cycle, or back-end tasks, as scoped
  • Exception logs for items that could not be completed from the information on hand
  • Payment-posting and charge-entry work inside the stated 24-hour turnaround, where that turnaround is in the statement of work
  • Audit-sample results you can reconcile to production

Quality approach

The 24-hour turnaround covers payment posting and charge entry. It does not automatically cover prior authorisation, coding, denials, or credit-balance research.

A dedicated quality team reviews a daily audit sample of 3–5%, with SOP sign-off and calibration.

Sampling is a control. It is not a published collection percentage, days-in-A/R figure, or denial rate. ForeMed does not publish those figures.

Questions

What is in each stage of the cycle?

Front-end is eligibility and benefit verification, and prior authorisation. Mid-cycle is charge posting and validation, and billing and edit resolution. Back-end is payment posting, credit balance, and denial management and A/R. Coding is a related service.

What does the 24-hour turnaround cover?

Payment posting and charge entry have a stated 24-hour turnaround. Other tasks take the time written in the statement of work.

Will you replace our billing system?

No. We work in the process and the systems you use. A change of system is your decision, not a condition of the engagement.

Can we engage only denials, or only eligibility?

Yes. The three stages describe the full cycle. A statement of work can cover one task.
