Drug safety support

Pharmacovigilance

Drug-safety support for pharmaceutical, biotechnology, and contract-research teams. We work inside your pharmacovigilance system. We do not take over the marketing authorisation holder’s legal duty.

ForeMed supports the operational work of pharmacovigilance: case processing, literature surveillance, aggregate reports, signal management, risk management plans, qualified-person support, safety-database tasks, and audits.

Every activity is done to your procedures, in the safety database you provide, on the products you assign. We do not claim a licence to a safety platform, and we do not describe ourselves as appointed QPPV unless a written appointment says so.

What we support

01

ICSR case processing

We support the individual case safety report from receipt through preparation for submission. The steps we can take on are intake, triage, data entry, MedDRA and WHO-DD coding, narrative drafting, quality check, and submission support.

Cases may be spontaneous, solicited, from literature, or from clinical trials, when your procedures include that source. Seriousness, expectedness, and causality follow your conventions. Medical review stays with the reviewer your procedure names.

  • Intake and triage
  • Data entry
  • MedDRA and WHO-DD coding
  • Narratives
  • Quality check
  • Submission support

02

Literature surveillance

We support screening of the literature sources and search strategies you approve, identification of reports that meet your case criteria, and a documented decision to process or to exclude.

Hits that meet your case definition are handed to ICSR processing. We do not claim a proprietary literature database.

03

Aggregate reports

We support drafting and update of periodic aggregate safety reports from the data cuts, line listings, and templates you provide.

Report types we support include PSUR/PBRER, DSUR, and PADER. Sign-off remains with the safety physician or other role your procedure names. We draft. We do not approve the benefit-risk conclusion in your place.

  • PSUR/PBRER
  • DSUR
  • PADER

04

Signal detection and management

We support signal detection and the path after a signal is seen: case-series review, validation, assessment, and tracking through to a decision you record.

Sources can include your case data, literature, and other datasets you make available. A signal is not communicated to an authority, an investigator, or the public except through the path in your signal procedure.

05

Risk management plans

We support risk management plans: drafting and update of safety concerns, pharmacovigilance activities, and risk-minimisation measures so they match the safety profile you hold.

Approval of the plan, and any commitment it creates, stays with your safety governance.

06

QPPV and local safety support

We support qualified-person and local safety activities: deputy and local-contact tasks, procedure maintenance, and the document chase that an inspection requires, where you have appointed people to those roles.

A listing on this page is not an appointment. A QPPV is named in your pharmacovigilance system and accepted by the relevant authority. ForeMed does not state that it holds that appointment.

07

Safety database support

We support day-to-day work in the safety database you license and host: case entry, workflow queues, and reconciliation tasks your administrators define.

ForeMed does not claim a licence to any safety database. Configuration changes that affect validation are proposed, not applied, unless your change-control says otherwise.

08

PV audits

We support pharmacovigilance audits of partners, affiliates, or internal processes: planning, sampling, interviews, and a findings report mapped to your procedure.

An audit report is a set of findings. It is not a certificate, and it is not an inspection outcome.

How a drug-safety engagement runs

  1. 01

    Read your system

Procedures, database, products, case sources, and the clock you already work to. We list the tasks you want supported and the tasks that stay with you.

  2. 02

    Train to your rules

Case definitions, coding conventions, narrative templates, and who signs medical review. Production waits until that training is recorded.

  3. 03

    Process assigned work

Cases, literature, reports, signals, or audit tasks, only inside the procedures you issued. Out-of-procedure items are escalated, not improvised.

  4. 04

    Quality check and governance

Quality check before a case or a report is offered back to you, plus a regular review of findings. Sample size is agreed per engagement. It is not assumed from the coding audit rate.

Deliverables

  • Cases processed to your procedure, ready for the review and submission path you name
  • Literature screening logs with process-or-exclude decisions
  • Draft aggregate reports (PSUR/PBRER, DSUR, PADER) from your data cut
  • Signal-tracking entries and draft risk-management updates
  • Audit findings reports, when an audit is in scope

Quality approach

Pharmacovigilance quality is the quality check in your procedure, plus a sample agreed before production. ForeMed’s 3–5% daily coding sample is a coding and revenue-cycle practice. It is not automatically the sample rate for drug safety.

MedDRA and WHO-DD coding is checked against the version you specify. We do not describe dictionary coding as a licence we hold on your behalf.

We will not call this work certified, inspection-proof, or a substitute for the marketing authorisation holder’s pharmacovigilance system.

Questions

Do you process cases in our safety database?

Yes, when you provide access. We work in the database you license. We do not require you to move cases onto a ForeMed platform, and we do not claim a database licence of our own.

Which aggregate reports can you support?

PSUR/PBRER, DSUR, and PADER, drafted from your data and templates. Other periodic reports can be discussed if your procedure defines them. Sign-off stays with your named reviewer.

Are you our QPPV?

Only if you make that appointment in writing and the relevant authority accepts it. This page describes support for qualified-person and local safety tasks. It is not an appointment.

Can literature or signal work be separate from case processing?

Yes. Each subsection on this page can be scoped on its own. Tell us the products, the volume, and the procedure, and we will limit the work to that.

Talk through pharmacovigilance

Name the setting, the volume, and which part of this page you want. We will scope from that, not from a standard package.
